Arrival
The patients attempt to get to the treatment center for the program, which proves to be very difficult for one OCD sufferer. Once at treatment, one patient fails to connect with the group, while another considers leaving when she finds out there's only one bathroom.
What's Your Problem
Dr. Tolin gives the group three days to continue their compulsions, while monitoring them so he can understand their triggers. In their first group session, one patient struggles to complete a seemingly simple task. In an exercise Dr. Tolin calls "Fear Movies" - one patient has a severe anxiety attack.
Today It Begins
Today is the day all patients must stop their compulsions cold turkey. Anxieties run high as the group attempts to deal with this- including not being able to take showers or wash hands. A field trip to Skid Row to face various fears drives one patient to the brink and they consider leaving the program.
Family Therapy
One contestant must decide if they will leave the program. For the group- this week is "Family Therapy." Dr. Tolin invites the families of the patients so that they can participate in exposures with their loved ones. It's an eye opening exercise for the families, and in the end, one family member must make a tough decision about whether or not to keep their loved one in the program.
The Hospital
Dr. Tolin is taking the entire group on an overnight field trip to an abandoned hospital. Here, they must face various OCD triggers- including death, HIV, Cancer, blood & fears of the supernatural. One patient is continuing to show signs of regression, and Dr. Tolin fears they may not be right for the program.
Extreme Exposures
Dr. Tolin asks one patient to leave the program. For the rest of the group, today begins the highest level of exposures. They have now reached the top of their "Fear Hierarchies" and the exposures are extreme. Some patients must spend a day in jail, and a mother must perform a funeral for her own son.
Home Environments
Dr. Tolin visits patients' homes to get rid of triggers in the home environment and to "contaminate" patients' safe zones.
Last Day Of Treatments And 3 Months Later Followups
Dr. Tolin holds a final group session, and the patients face their toughest exposure again.
